ABOUT THE OFFICE

HUMAN RESOURCES - ACTIVE EMPLOYEE RESOURCES

Due to the complexity of instructions, some forms may only be obtained in the Human Resources Office. If you do not see the forms you need or have additional questions, please contact a member of the Human Resources Staff for assistance.

General Forms

Please complete the applicable form if you wish to change your tax withholding or direct deposit account, and forward to the Payroll Officer.

Change of Address Forms

These forms will all need to be completed any time an employee changes their address.  On the Health Insurance form and the Minnesota Life form, please fill in only the employee information sections, sign and date.  All sections of the other forms should be completed.  Forward these completed forms to the HR department.

Arkansas Public Employees Retirement System (APERS)

Citistreet 457b Retirement Savings

The Arkansas Diamond Deferred Compensation Plan (457b) is a voluntary retirement savings plan separate from APERS that allows payroll deduction contributions with pre-tax dollars.  As an employee, you may choose a voluntary dollar amount to be payroll deducted up to the annual contribution limits set by the IRS. For 2008, this limit is $15,500.

Employee Benefits Division (EBD) / Health Insurance

If you have or have had a “Qualifying Event”* and plan to change your coverage, please notify the HR office immediately.  Changes to your plan can only be made in the first 30 days after the event.
*Marriage, divorce, birth or adoption of a child, loss of other coverage etc.

NMHC RX

NMHC (National Medical Health Card) manages the prescription drug program for Arkansas employees.  In addition, NMHC Mail program provides home delivery of prescription medications.

American Health Holding

EBD has contracted with American Health Holding to provide utilization management services including precertification, predetermination, and concurrent review.

Fringe Benefits Management Company (FBMC)

Copies of the 2008 Cafeteria Plan Benefit Guide are available in the HR Office. These guides are provided on an annual basis by FBMC. To obtain a copy, please email the hr office at hrsos@sos.arkansas.gov or contact an HR Staff member.

Datapath Administrative Services HAS

This service may only be established by enrolling in the high deductible health plan.

“Changing behaviors and improving lives” Corphealth is changing their name to LifeSynch in November.

Voluntary/Supplemental Insurance
